What was found
On inspection, the drain line was found to be clogged. And because it had been clogged for a while, a second, related condition had developed: water had accumulated on the evaporator coil and frozen there. This is a classic chain of events. A frost-free refrigerator periodically defrosts its evaporator coil, and that meltwater is supposed to run down the defrost drain line and away. When the drain line clogs, the meltwater cannot drain, so it backs up, and near the cold coil it refreezes, building up ice on the coil, while excess water can leak out, which is the small leak the customer noticed from the middle of the unit. So the clog is the root cause, producing both the ice on the coil and the leak. Clearing the drain line and defrosting the coil are what is required to restore proper operation, addressing both the cause and the ice it created.
